United Way has selected Project G.R.O.W.T.H. Initiative
to give away over 1000 toys.
In order to participate in the giveaway, we are requesting that you please fill out both the Terms and Liability Disclosure and the Event Participation forms using either the Button below or using the drop-down menu under the Toy Giveaway December  2019 with a valid email address and phone number. 

We will email you should you be selected to attend the event, we ask that you please not bring any extra guests with you. Upon your acceptance email, you will need to gather the necessary documents:

  • Please bring a valid ID

  • current utility bill (must be within the last 28 days)

  • proof of social security award 

  • veteran’s benefits award letter 

  • TANF Approval Letter

  • Snap Approval Letter 

  • Unemployment Award Letter

 

If you have incomplete or missing information, you will no longer be eligible and will be asked to leave the event. You must fill out both forms completely in order to register. This form is not compatible with mobile devices, please register from a computer.
